Skilled Rehabilitation

If you no longer need acute care services, but need more help before you can care for yourself at home, the Skilled Rehabilitation Program is for you. The Skilled Rehabilitation Program at MDI Hospital allows your physician to change or “swing” your care from acute care to short term skilled care. In the Skilled Rehabilitation Program, you receive daily therapy to meet self-care goals set by you and the Skilled Rehabilitation Team. You recover close to home, in the familiar surroundings of your community hospital.

Skilled Rehabilitation care involves not only your doctor, but an interdisciplinary team of providers who establish and oversee a care plan that will meet your rehab goals for discharge.

In the Skilled Rehabilitation Program you are encouraged to wear your own clothes, eat your meals in the Activities Room, and take part in a variety of daily activities. You receive therapy to meet your individual needs.

To qualify for admission to the Skilled Rehabilitation Program you must have at least a three-day stay as an acute care patient and must require a “skilled” level of care.
