Coastal Care Team Social Worker

Be on the leading edge of healthcare delivery in your community.  MDI Hospital is leading an innovative, grant funded pilot project to assist patients with chronic illnesses better manage their condition and improve their wellbeing. This team works in conjunction with the Patient Centered Medical Homes and MaineCare Health Homes throughout Hancock County, Eastern Waldo County and Western Washington County to provide extra support to those who struggle with multiple emergency room visits and hospital admissions. The position entails advocacy skills and knowledge of behavioral health and community resources. This position will maintain absolute confidentiality of all patient information and abide with all policies and procedures of the MDIH /CCT. Bachelors degree in Psychology or Social Work. Ideal candidate will have 3-5 years experience in social work.  Experience with electronic health records, home health, chronic illness, motivational interviewing, goal setting, and providing constructive feedback is preferred along with knowledge of local community resources. Candidate must have strong organizational and time management skills. 10/28/14

Director of Care Management

Full Time, 40 hours per week, Benefit Eligible. The Director of Care Management participates in major programming for holistic care throughout the community; serves as the leader for the activities of the care management and social work staff; oversees the coordinator of the federally designated diabetes program and serves as a leader for primary care activities in collaboration with senior management, medical staff and community participants. Master level preparation is strongly encouraged. Current Maine license is required. Membership in relevant professional organizations is expected. The Director demonstrates proficiency in public health issues and should have experience in the planning and implementation of care for groups of people either in the hospital or community setting. The Director should be well versed in Medical Staff relations and have a proven record of collaborative practice. Training in care management, leadership principles, advanced nursing care or social work is needed. The Director must have the vision and drive to function as an effective change agent while improving the care of groups of patients. Recent changes in the organization such as the construction of a new SW Harbor facility and development of Senior services at Birch Bay and SW are examples of innovation that should continue. Has the ability to plan, teach, implement and evaluate. A quick thinker who needs minimal direction is preferred.

RN Critical Care and MS Telemetry

Full time days, 36 hours per week, in both Critical Care and Med Surg Tele, including every other weekend.  Responsible for direct total nursing care of patients including assessment, planning, implementation and evaluation within the policies and procedures standard in the Critical Care Unit. The CCS RN will plan and deliver care for acute adult and geriatric medical and surgical patients, coronary patients, and in rare cases, infant, pediatric and adolescent patients. The CCS RN will provide after hours post-anesthesia recovery care. The Telemetry RN will provide care for those patients assigned to his/her unit. The CCS RN will maintain skills in detection and management of all aspects of intensive care, including use of specialized equipment and procedures. Must have ACLS and be eligible for PALS within one year.  Current Maine licensure as a registered professional nurse, CPR Provider Status, and Critical Care/Code Blue Course required.  Previous experience preferred including recent critical care experience and recent training/experience in medication administration and nursing process in an acute care setting.

Full time, 40 hours a week, benefit eligible position.  Provides technical oversight to the laboratory, ensuring accurate and timely delivery of laboratory results. Through training and supervision, the laboratory manager ensures that laboratory personnel adhere to best documented practices, including quality, safety and environmental procedures. He or she maintains, audits, and monitors all required systems, processes, methods and equipment. The incumbent collects specimens, performs quality control and analytical testing, and evaluates and reports results following theories and principles of prescribed strategies. He or she uses independent judgment in problem solving and the evaluation of abnormal results, and is available as a resource for MTs, MLTs and phlebotomists. Will assist Director of Laboratory with staff evaluations and budget planning.  Bachelor's degree in Medical Technology required.  Registry with ASCP, AMT or NCA or registry eligible.  Clinical experience in high complexity testing area required. 8/20/14
