Birch Bay Retirement Village is hiring a Memory Care Activities Coordinator to work in our Memory Care unit.

 Qualified Applicant will be responsible for:

-executing activities for seniors living with memory loss and limited mobility

-transporting residents in our 15 passenger van

-supporting resident social needs

-documenting resident activities

Previous experience working with seniors with memory loss. Activities certificate preferred.

RN Care Manager/Discharge Planner

Temporary Full Time, 36 hours per week, Day Shift, Nov. 6, 2014 - Mar. 6, 2015.  No weekends/holidays.  RN Provides timely coordination of quality healthcare services to meet individual’s specific healthcare needs in a cost-effective manner.  Provides a “collaborative process which assesses plans, implements, coordinates, monitors and evaluates options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes.” (CMSA definition of Case Management.)  Provides and coordinates care with patients and involved persons for an appropriate discharge plan that meets the medical, rehabilitative and financial needs of the individual at the time of discharge from the hospital.  Provides pre-op teaching through orthopedic office.  This is a telephonic care position which will also require floating to inpatient care coordination.  The ideal candidate must show proficiency in all technical/computer skills required to perform their work.  Proficiency includes use of eCW, Maine Health Alliance and other software applications.  Minimum of two years of experience in the field of licensure, demonstrated knowledge of health, social service, and funding sources.  Care management experience preferred.  Emotional maturity and good people skills required.  Good focus on standards and ethics.  Ability to multitask, prioritize and organize.  Ability to form collaborative, proactive and patient-focused relationships to focus, facilitate and maximize client healthcare outcomes.  Demonstrated communication and leadership skills with healthcare team members to facilitate maximal client outcomes.

Provides timely care coordination of quality healthcare services to meet individual specific healthcare needs in a cost effective manner.  Care Manager with primary role of Inpatient Care Manager in combination with coagulation clinic.  Current Maine licensure required.  Suitable candidate must maintain continuing education appropriate to care management and professional licensure.  Anticoagulation therapy management certificate required within 3 months.  Current BLS required.  Minimum of 5 years acute care.  Medical Surgical experience required.  Care management experience required.  Must be willing to cross train to other areas.  Weekend/Holiday call coverage may be required.

Manager - Lab

Full time, 40 hours a week, benefit eligible position.  Provides technical oversight to the laboratory, ensuring accurate and timely delivery of laboratory results. Through training and supervision, the laboratory manager ensures that laboratory personnel adhere to best documented practices, including quality, safety and environmental procedures. He or she maintains, audits, and monitors all required systems, processes, methods and equipment. The incumbent collects specimens, performs quality control and analytical testing, and evaluates and reports results following theories and principles of prescribed strategies. He or she uses independent judgment in problem solving and the evaluation of abnormal results, and is available as a resource for MTs, MLTs and phlebotomists. Will assist Director of Laboratory with staff evaluations and budget planning.  Bachelor's degree in Medical Technology required.  Registry with ASCP, AMT or NCA or registry eligible.  Clinical experience in high complexity testing area required. 8/20/14
