Enroll At Your Next MDI Hospital & Health Center Visit
If you haven’t seen us since we launched our new electronic health record system, some personal demographic and medical information may not be available at your first medical visit. To help us better serve you, we encourage all patients with an appointment on or after March 7, 2022 to:
- Arrive 30 Minutes Early
- Bring a List of All Prescription and Non-Prescription Medications
- Bring Current Insurance & Identification Information
The Patient Portal offers a convenient messaging system which is a secure communication platform to view your medical record and to connect with your healthcare team. It should not be used for non-health-related matters, urgent issues, or in the event of a medical emergency.
Our goal is to respond to any messages sent to a clinic or provider with 3 business days. If you do not receive a response within that time period, please contact your provider’s office directly by phone, and if you are experiencing a medical emergency, please call 911.
